Medical Record Review for Law Firms: In-House, Outsourced or AI
Medical record review for law firms comes in four forms: a paralegal in-house, an outsourced nurse or physician review service, a litigation platform with a review module, or a compliant AI workspace. This guide compares them on cost, security and the question volume your case will generate. Hathr.AI is the fourth option, and you can see the full product page at AI medical record review on Hathr.AI.

Medical record review for law firms: the four options compared
The right option for a law firm depends on production size, handwriting share and how many questions the case will generate after the first summary. The table below gives a verdict on each criterion instead of a list of trade-offs.

Do law firms need a BAA for medical record review?
A law firm needs a BAA for medical record review when the firm acts as a business associate, which HIPAA defines to include legal services performed for a covered entity under 45 CFR 160.103. Defense counsel for a hospital, a physician group or a health plan falls in that group, and every vendor downstream of the firm needs a BAA too.
A plaintiff firm holding records under the client's own authorization, or under a qualified protective order described in 45 CFR 164.512(e), is generally not a business associate. Its obligations run through the protective order, state privacy law and the duty of confidentiality in ABA Model Rule 1.6.
We take a firm position on this. Treat every production as if a BAA were required and buy on that basis. It costs nothing when the BAA turns out to be unnecessary, and it removes the one decision that depends on a posture analysis being right. This describes how the frameworks fit together. It is not legal advice, so confirm your posture with privacy counsel.
What does a medical record review include for attorneys?
A medical record review for attorneys includes a dated chronology with page citations, a summary of injuries and treatment, the pre-existing conditions the record documents, treatment gaps, and a reconciliation of billing against clinical records. Deposition summaries and causation questions sit on top of that base.
The chronology carries the most weight. Our medical chronology template lists the twelve fields we recommend, including the litigation fields most templates leave out: source document ID, legibility and confidence, conflict flag and gap to next treatment. If you are comparing chronology software alone, our AI medical chronology buyer's guide covers that narrower purchase. If you want the definition and the preparation steps first, start with what a medical chronology is and how to prepare one.
Retrieval is a separate job. Getting the records from custodians, paying statutory copy fees and chasing missing providers belongs to a retrieval vendor, and our guide to medical records retrieval for lawyers covers the four questions to ask one.
How much does medical record review cost a law firm?
Medical record review costs a law firm whatever the pricing unit multiplies. Services charge per page or per case, so a 12,000-page production costs about twenty times a 600-page one. A per-seat tool costs the same for both. Run the arithmetic with four numbers you already have:
The fourth number decides it. A production reviewed once by a service for one chronology is a single invoice, and often a fair one. The same production asked forty questions over eighteen months (every mention of the left shoulder, every provider who documented a prior injury, every place the record contradicts the deposition) turns into forty requests to a service and forty queries in a flat-rate workspace. Our breakdown of medical record review services versus software walks through a full worksheet.
Try it on a closed matter: upload one production you already know, ask Hathr.AI for a dated chronology with page citations, then ask three questions you can already answer. Start the 7-day free trial and run that test on real records, since the BAA is in place from signup.
What published research shows
A 2025 validation study in BMJ Open compared physician-written and LLM-written summaries of 50 patient records. Physicians took about 7 minutes per summary. The model drafted each in 15.7 seconds, and evaluators found no significant difference in completeness or correctness. The study timed the draft, not the check, so plan for your reviewer's verification hours. On a production you will put in front of opposing counsel, page citations give that reviewer a source for every statement, which is why Hathr.AI returns the page behind each answer.
We also see what firms ask before they buy. Over the last six months, our own Search Console shows queries such as "legal AI platform that doesn't retain medical data after processing" (211 impressions) and "legal AI with a business associate agreement" (80 impressions) reaching hathr.ai. Firms ask about data handling before features. We answer that first below.
Is AI medical record review secure enough for a law firm?
AI medical record review is secure enough for a law firm when the vendor signs a BAA, names where the records are processed, and commits in writing to never training on them. Get each answer in writing from the vendor, because a setting you toggle in an app can change with the next release.
Hathr.AI gives the same four answers for a two-lawyer firm and a national practice:
ABA Formal Opinion 512 asks lawyers to understand a generative AI tool's handling of client information before using it. Those four points are the answers to put in your file.
Where an outsourced reviewer fits better
An outsourced nurse or physician reviewer fits better when you need a clinician's signature on the summary, when the production is small and largely handwritten, or when the case turns on a standard-of-care opinion. Hathr.AI drafts the cited output. A reviewer verifies the flagged entries, and a professional decides what the record means.
Many firms use both. They run the full production through a compliant AI workspace to build the index and answer the volume questions, then send the twenty pages that matter to the physician reviewer. That split cuts the reviewer's invoice to the part only a clinician can do.

Frequently asked questions
I need legal AI with a business associate agreement for HIPAA compliance. What platforms offer this?
Many legal AI tools do not sign BAAs, because their core market is documents in general. Hathr.AI includes a signed BAA on every web app account, accepted electronically at signup, with no seat minimum, so a two-person firm gets the same contract as a national practice.
Which AI tools for summarizing medical records are most used by law firms?
Law firms use three kinds: chronology products built into litigation platforms, AI-assisted review services that pair software with nurse reviewers, and compliant AI workspaces such as Hathr.AI. Pick by your bottleneck. Matter integration favors the platform, a signed clinical summary favors the service, and high question volume favors the workspace.
Does a plaintiff personal injury firm need a BAA?
A plaintiff firm holding records under a patient authorization or a qualified protective order is generally not a HIPAA business associate. A firm doing legal work for a covered entity generally is. Treating every production as if a BAA were required costs nothing and removes the risk of a wrong posture analysis.
How much does medical record review cost for attorneys?
Services price per page or per case, so cost rises with production size and every follow-up question. Hathr.AI costs $47 per user per month with no per-page fee and no seat minimum, as of August 2026. Model both against your monthly volume and your follow-up questions per matter.
Can AI replace a legal nurse consultant?
No. AI drafts the cited chronology and answers questions against the file. A legal nurse consultant or physician reviewer still verifies flagged entries and gives the clinical opinion. The hours move from transcription to verification and analysis.
Does Hathr.AI keep my clients' medical records?
The models Hathr.AI uses retain nothing at the model layer. Documents you upload stay in your account, segmented from other customers, until you delete them, and you can delete them at any time. Hathr.AI never uses customer data for model training.
