Hospice Documentation: The Complete 2026 Guide to Certification, Narratives, and Audit-Readiness
Hospice documentation exists to prove one thing to Medicare: that the patient has a life expectancy of six months or less if the illness runs its normal course, and that the record shows it. Get the documentation right and claims pay and survive review; get it wrong — a missing narrative, a late face-to-face, a decline no one wrote down — and the same clinically eligible patient triggers a denial. This guide covers what belongs in the record, how the physician narrative has to read, and how to keep a chart audit-ready. It starts from Medicare’s hospice eligibility criteria, which define the clinical bar the documentation has to demonstrate.

The certification packet
Every hospice stay is built on a small set of required documents:
- Election statement: the patient or representative elects the hospice benefit and acknowledges the palliative focus for the terminal illness.
- Certification of Terminal Illness (CTI): for the initial benefit period, the hospice medical director (or a physician member of the interdisciplinary group) and the patient’s attending physician certify the six-month prognosis; subsequent periods require recertification.
- Benefit periods: two 90-day periods followed by an unlimited number of 60-day periods, each requiring recertification.

The physician narrative
The narrative is where most documentation fails. Medicare requires a brief explanation, written by the certifying physician, of the clinical findings that support a life expectancy of six months or less. It must be the physician’s own composition — not a checkbox, not a form phrase, and not a paragraph indistinguishable from every other patient’s. Practical rules:
- Explain the why: connect specific findings (functional decline, weight loss, infections, admissions) to the prognosis.

Tracking functional decline
Prognosis in hospice is a trajectory, and the record has to show movement. Capture a validated functional measure — the Palliative Performance Scale (PPS), Karnofsky (KPS), or FAST stage for dementia — at admission and at each recertification, alongside serial weights and ADL dependence. A single PPS of 40% means little; a PPS that fell from 60% to 40% to 30% over three periods documents decline. Note infections, ER visits, and hospitalizations as they occur so the pattern is already in the chart when a reviewer asks.
Building an audit-ready record
An Additional Documentation Request (ADR) asks the hospice to prove eligibility from the record alone. To be ready:
- Keep the decline story consistent across disciplines — the nurse, social worker, and physician notes should not contradict each other.
- Ensure every certification, narrative, and F2F is signed and dated before billing.
- Make the clinical evidence findable: the indicators that support the diagnosis-specific prognosis should be easy to locate, not buried.
- Reconcile eligibility with the financial picture — sustained long lengths of stay draw scrutiny and interact with the aggregate hospice cap calculation.
